Aftercare Program

Best Suited For:
Children, ages 10-17, who have been successfully discharged from placement not limited to the Children's Home of Jefferson County.

The goal of the Aftercare Program is to provide services that support a smooth transition for the child and family from residential care to their home and community.

Family Assessment
This evaluates the family, the impact the child ’s difficulties have on the family, and how the family copes with their environment. This assessment ensures that the child is in a stable and healthy environment.

Aftercare Plan
This is a group effort between the CHJC worker, DSS caseworker, child, family, and community to work towards the successful return of the child to his/her home community. Together, a plan is developed to fit the individual child and family needs. This continual support enables the child to be successful.

Visitation with Family
These weekly visits evaluate the progress within the family, help identify problem areas and solutions in their life, and help the family use their strengths to work as a family unit. This provides the family with an overall understanding of their child.

Educational Liaison
Every child receives weekly contact with an educational liaison. The liaison monitors the child's progress and involvement in school to ensure the child's educational success.
