Adult Case Management Services

The Adult Case Management Services' goal is to reduce hospitalization and excessive hospital visits for mental health concerns while introducing clients to community services and crisis management programs.  Case Managers work with clients in developing and utilizing support networks that assists them in achieving and maintaining their functional goals.
Best Suited For:
Adults 17 and over:
  • Are seriously and persistently mentally ill, are considered a high-risk for inpatient hospitalization, and have had multiple admissions to inpatient and outpatient psychiatric programs
  • Are seriously and persistently mentally ill and experience symptoms that are difficult to meet in the existing mental health care system
  • Have a serious and persistent mental illness and have a history of excessive use of crisis services
  • Have multiple disabilities including a primary diagnosis of a mental illness
  • Are seriously and persistently mentally ill and are considered to be a 'forensic' client

Referral Process:
The Single Point of Entry has a no exclusion policy.  Any individual who is experiencing mental health problems or any agency working with clients with mental illnesses are welcome to submit a referral to the Committee.

Once a referral is received, the director and team leader gather intake information to present to the the screening committee, which meets monthly.  The screening committee reviews the intake information and makes a recommendation as to appropriate services.  This could be Case Management Services or other community programs; nevertheless, all referral are reviewed and referred to appropriate programs.
